Our team of fourteen retina surgeons are committed to providing timely, effective and compassionate care, while keeping your practice informed of our findings and progress along the way. We look forward to serving you, your practice, and your patients.

Our specialists at Colorado Retina Associates treat an extensive range of common to complex vision-threatening eye conditions and diseases. Our practice is operated by physicians with multiple specialties working in tandem to provide exceptional medical and surgical retinal care. Our multidisciplinary team is better equipped to focus on the individual patient, guiding them through the diagnostic testing process, into treatment, counseling, maintenance, and hopefully recovery. With 2-3 physicians working in each of our five Denver Meto clinics, peer-to-peer physician collaboration allows our teams to provide each patient options and accurate information needed to make the best individual decision for their own healthcare. In addition, our practice is proud to invest in advanced equipment and technology to ensure an expedited, sound diagnosis for prompt treatment.
Our Process Is Easy
STEP 1: Submit your referral through you preferred method: website, fax, email, or our Phreesia portal. Don’t forget to include the patients’ phone number for our team to call and schedule.
STEP 2: In addition, if possible please include with a referral cover sheet additional supporting patient files and imaging including:
- Patient Demographics
- Copy of Insurance Cards
- Most Recent Exam/Progress Notes
- OCT/FA Images
For same-day or urgent referrals, please call our triage line ASAP at 303-261-1600 x2100.
If you do not wish to submit a referral online, please fax all forms and information to (303) 261-1601.
What to expect after submitting your referral:

For non-urgent referrals allow up to 2 business days for us to process your request. We will reach out to your patient directly to schedule based on your noted preferences and urgency of potential diagnosis.

For urgent or emergent referrals call us ASAP at 303-261-1600, and follow the prompts for providers or triage - ext. 2100. Depending on symptoms, all urgent referrals will either be seen same or next day. We also provide a Saturday clinic for those Friday night scares.

If for whatever reason we are unable to reach your referred patient after three attempts, our team will notify your office.

Post-appointment, CRA physician exam notes will be faxed to your office for your records.
